We DO NOT participate in EAP, Medicare (i.e. AHCCCS), Medicaid

Adrienna Wilson: Aetna (via Headway) . Superbill is available when requested in advance.

Alexandra Trusty: Aetna (via Headway) . Superbill is available when requested in advance.

LaQuita Dorsey: Aetna (Phoenix Counseling LLC) , BCBS (Under Michiko Pepple). Superbill is NOT offered.

Michiko Pepple: Aetna (via Headway) , BCBS, United Healthcare. Superbill is available when requested in advance.

We DO ACCEPT many of the following commercial plans.

Private Pay Rates

All fees are due at the time of service.

All Initial Appointment: $190

Follow up appointment:

$170 with LPC/LCSW providers

$150 with LAC/LMSW providers

We do not file claim for out of network plans.

If you plan to file a claim with a insurance which they do not accept, please let us know when you make your initial appointment as some providers offer a superbill.

Cancellation Policy

Please provide a minimum of 24 hours notice if you would like to reschedule or need to cancel a session. Failure to provide at least 24 hours advance notice or missing a session will result in a late-cancellation fee of $150, which will be charged on your credit card automatically. This is not the fee for service, but the cancellation fee and will not covered by any insurance plans.
